For some reason the usb bluetooth adapter is not turn on when the laptop starts.
There are two ways of doing that:

ACPI enabled kernel
http://www.sfires.net/toshiba/
ACPI enabled kernel(other, supports
3ghsdpa radio&power switch,bluetooth radio&power switch,wlan radio switch): http://linux.reggie.hu/toshiba_acpi
Non-ACPI enabled kernel
http://www.holtmann.org/linux/bluetooth/toshiba.html

After this is done the bluetooth adapter is turned on and you can follow the Gentoo Linux Bluetooth Guide

Bluetooth can be enabled using toshset util:

# toshset -bluetooth on
